1. Fill out Ez Sleep’s SHQ / Prescription Form

Patient fills out our simplified Sleep Health Questionnaire and quickly identifies if they are at risk for sleeping disorders.

2. Patient Consultation

Medical Examiner speaks with patient about potential sleeping disorders identified with Ez Sleep SHQ/Rx.

3. Order Ez Sleep HST

Doctor signs Sleep Health Questionnaire / Rx form and sends it to Ez Sleep In-Home Testing for processing.

4. Benefits Verification, HST Education & Schedule

Ez Sleep verifies benefits — then calls patient to educate on the risk of sleep apnea, untreated outcomes, Ez Sleep’s process and schedule their in-home sleep test.

5. Ez Sleep HST is shipped

Ez Sleep mails the patient’s HST to their home with a prepaid return shipping label included.

6. Patient takes Ez Sleep HST

Patient performs prescribed HST, with access to instructional guide, video and 24hr tech support. Once complete, the device is sent back via USPS with Ez Sleep prepaid shipping label. The entire process usually takes 7-10 days.

7. Diagnosis & Treatment Recommendation Reports

Ez Sleep Clinical Team analyzes the study and sends you a simplified Summary Diagnosis & Treatment Recommendation Report that outlines key sleep data and applicable treatment options, as well as detailed Multi-Night Comparison Reports with enhanced scoring data. Additionally, Ez Sleep provides innovative data driven products for advanced co-therapy like myTAP™ Temporary Trial Appliance.
